Worksheets

(A few people have reported problems with the new version of the worksheets.
Please test printing before entering all your information. If you have any trouble
you can download the older worksheets here).

You can now enter your numbers directly into the worksheet.
After downloading, click in the right column to insert your numbers.
You can tab to move to the next row in the column.
When you're done, print it to bring to your appointment.

Note: You may not be able to save the file with your changes, only print it.
(Even though security is set to 'none'. Seems to be an Acrobat versions
compatability issue.) If you're on a Mac using Preview you should have no
trouble saving, Save As.
